Onychomycosis. The most common location for onychomycosis is the big toenail. The infection starts distally where the dermatophyte grows under the distal end of the nail and on the skin beneath the nail, and from there it may spread to the whole nail plate damaging it. However, the fungus will not damage the germinal matrix, the tissue generating the nail plate.
