Differential Diagnosis of Mole and Melanoma

The ABCD checklist and the revised 7-point checklist to distinguish melanoma from benign lesions may be fairly accurate diagnostic aids. Level of evidence: "C"

A systematic review 1 including 12 studies was abstracted in DARE. 2 studies evaluated examiner's precision for specific features of benign pigmented lesions; 4 studies assessed the accuracy of the checklists (557 lesions); 12 studies evaluated the accuracy for detecting the presence or absence of melanoma (10,936+ lesions).

Two studies found the sensitivity for the ABCD checklist to be 92% (95% CI 82% to 96%) and 100% (95% 54% to 100%), and one study found the specificity to be 98% (95% CI 95% to 99%).

The 7-point checklist has been reported to have a sensitivity of 79% (70% to 85%) to 100% (94% to 100%) and a specificity of 98% (95% to 99%). The physician's global assessments for detecting the presence or absence of melanoma are estimated to have a specificity of 96% and a sensitivity of 50% to 97%.

Comment: The quality of evidence is upgraded by large magnitude of effect but downgraded by study limitations.
