Removing Clavus with Scalpel from Diabetic Foot Ulcer

A 62-year-old man with a long history of poorly controlled type 2 diabetes had a large neuropathic foot ulcer. The ulcer cavity is larger than the diameter of the visible ulcer. Thickened skin (clavus) on the margins of the ulcer increases the pressure over the underlying tissue, impeding healing. The thickened skin can be excised with a scalpel, avoiding to cut so deep that bleeding becomes visible. At the end of the video the tip of the scalpel points at visible blood as a sign that target depth has been reached. The patient was treated with a cast boot.
