Recommendations
Delay umbilical cord clamping for at least 3060 s in term and preterm infants, except when immediate clamping is necessary because of neonatal or maternal indications (ie, resuscitation).
Do not perform cord milking for extremely preterm infants (<28 wk of gestation).
There is insufficient evidence to support or refute umbilical cord milking in infants born at 32 wk of gestation or more, including term infants.
Practice Pearl
A 2019 study of umbilical cord milking was halted early because extremely preterm infants (2327 wk of gestation) in the cord milking arm more often developed intraventricular hemorrhage compared with similar infants in the delayed cord clamping group.
