Your discharge teaching should aim to ensure that the patient: - understands the illness
- understands and can manage medication modifications, schedules, and information
- carefully follows the ordered diet
- manages the activity level
- understands ordered treatments
- recognizes the need for rest
- knows about possible complications
- knows when and where to seek follow-up care
- is aware of available community resources (as needed).
Remember that your discharge teaching must include the patients family members and other caregivers to ensure that the patient receives proper home care.8,9,10,11,13,14,15,16 |
