Your discharge teaching should aim to ensure that the patient:

• understands the illness
• understands and can manage medication modifications, schedules, and information
• carefully follows the ordered diet
• manages the activity level
• understands ordered treatments
• recognizes the need for rest
• knows about possible complications
• knows when and where to seek follow-up care
• is aware of available community resources (as needed).

Remember that your discharge teaching must include the patient’s family members and other caregivers to ensure that the patient receives proper home care.^{8,9,10,11,13,14,15,16}
