Activate rapid response team (RRT) at first sign of decline in Pt status.

• Establish and support ABCs-Treat life-threatening emergencies immediately.
• Position Pt to protect airway for decreased LOR; position of comfort if Pt alert.
• Administer oxygen as indicated/ordered, and titrate to SpO₂ >92%.
• Notify HCP (using SBAR format) for any adverse changes in Pt status, including pertinent assessment findings and initial interventions.
• Establish IV, and administer IVF as ordered and titrate to maintain SBP >90 mm Hg.
• Obtain labs, ECG, and imaging or other diagnostic studies as ordered.
• Transfer to ICU if condition continues or worsens.
• Document assessments, any interventions, SBAR, and outcome.
