Dose Range/Adult Daily Dose

• Range: 150–800 mg/d
  Schizophrenia: Start 25 mg po 2 × daily, gradually increase to 300–400/800 mg/d.
  Bipolar mania: Start 100 mg/d po 2 divided doses, up to 800 mg/d (incremental)
  Bipolar depression: Start 50 mg, up to 300 mg by day 4.

Use/Common Side Effects (CSE)

• Use: Schizophrenia, Bipolar I mania: (monotherapy and adjunct to lithium/divalproex (bipolar maintenance); Bipolar I and II: acute depressive episodes
• CSE: Dizziness, headache, somnolence, weight gain, NMS, SEIZURES

Geriatric & Dose Considerations

• May require dose reduction; use cautiously in Alzheimer's, pts ≥65 y, & hx seizures. Warning: ↑ mortality in elderly with dementia-related psychosis. Also caution w CV/hepatic disease.
