Dose Range/Adult Daily Dose

• PO (Adults): Start: 10 mg once/d, may ↑ to 20 mg once/d; some clients may only tolerate 5 mg/d. For CYP2D6 poor metabolizers; CYP2D6 inhibitors; CYP2D6 strong inducers: see product insert for dosage adjustments. If daily dose is 15–20 mg/d, do not discontinue abruptly; taper to 10 mg/d for 1 week before discontinuing

Use/Common Side Effects (CSE)

• Use: Major Depressive Disorder.
  CSE: constipation, nausea, sexual dysfunction, dizziness, bleeding, hyponatremia (with SIADH). SEROTONIN SYNDROME; allergic reactions including ANGIOEDEMA

Geriatric & Dose Considerations

• Caution with dizziness in the elderly (monitor). Caution with bleeding especially with NSAIDs and aspirin. Monitor electrolytes (hyponatremia).

Assessment Cautions

• Assess: May trigger an angle closure glaucoma attack; monitor for mania/hypomania activation; electrolytes for hyponatremia; abnormal bleeding. Monitor for improvement or worsening symptoms, suicidal ideation or behaviors.
  Cautions: Serotonin syndrome with other serotonergic agents
  Children—young adults: Monitor closely for suicidal thoughts or behaviors.
  Contraindicated: Concurrent MAOIs or within 14 days of stopping MAOI.
