Geri: Appears on Beers list. risk of anticholinergic adverse reactions in older adults, including falls, delirium, and dementia.

CV: arrhythmias, hypertension, hypotension, palpitations

EENT: blurred vision

GI: dry mouth, constipation

GU: urinary hesitancy, urinary retention

Neuro: sedation, dizziness, excitation (↑ in children)

Interactions ⬆ ⬇

Drug-drug:

Additive CNS depression with other CNS depressants, including alcohol,opioids, and sedative/hypnotics.
Additive anticholinergic effects with otherdrugs possessing anticholinergic properties, including antidepressants, atropine, haloperidol,phenothiazines, quinidine, and disopyramide.
MAO inhibitors intensify and prolong anticholinergic effects of antihistamines.

Drug-Natural Products:

Concomitant use of kava, valerian, skullcap, chamomile, or hops can ↑ CNS depression.
↑anticholinergic effects with angel’s trumpet, jimson weed, and scopolia.

PO (Adults and Children ≥12 yr): 2.5 mg every 4–6 hr (not to exceed 10 mg/24 hr).
PO (Children 6–11 yr): 1.25 mg every 4–6 hr (not to exceed 5 mg/24 hr).

Oral solution: 0.25 mg/mLOTC; 0.625 mg/mLOTC; 0.938 mg/mLOTC; 1.25 mg/mLOTC
In combination with: pseudoephedrine OTC. See Appendix [not included in this PDA edition].

Assessment ⬆ ⬇

Assess symptoms (rhinitis, conjunctivitis, hives) prior to and periodically throughout therapy.
Assess lung sounds and character of bronchial secretions. Maintain fluid intake of 1500–2000 mL/day to decrease viscosity of secretions.

Lab Test Considerations:

May cause false-negative reactions on allergy skin tests; discontinue 3 days prior to testing.

Implementation ⬆ ⬇

PO: Administer oral doses with food or milk to ↓ GI irritation. Use calibrated measuring device to ensure accurate dose of syrup.

Patient/Family Teaching ⬆ ⬇

Instruct patient to take as directed. If a dose is missed, it should be taken as soon as remembered unless almost time for next dose.
Instruct patient to contact health care professional if symptoms persist.
May cause drowsiness. Caution patient to avoid driving or other activities requiring alertness until response to drug is known.
Caution patient against using alcohol or other CNS depressants concurrently with this drug.
Advise patient that good oral hygiene, frequent mouth rinses, and sugarless gum or candy may help relieve mouth dryness. Health care professional should be notified if mouth dryness persists >2 wk.
Rep: Advise females of reproductive potential to notify health care professional if pregnancy is planned or suspected and to avoid breastfeeding during therapy.
