Cyclophosphamide + Doxorubicin + 5-Flourouracil (CAF) 1,43- Prechemo: CBC, Chem, LFTs, and CA 2729. MUGA scan. HT3 and dexamethasone 20 mg. Moderately to highly emetogenic.
- Cyclophosphamide 600 mg/m2: IV in 5001000 cc NS on day 1.
- Doxorubicin 60 mg/m2: IV push on day 1. Potent vesicant.
- 5-Flourouracil 600 mg/m2: IV push on day 1.
- Side effects: Cyclophosphamide can cause rhinitis and irritation of nose and throat. Myelosuppression may be severe. G-CSF recommended. Nausea and vomiting, acute or delayed. Stomatitis and diarrhea can be severe and dose-limiting. Pericarditis-myocarditis syndrome. With cumulative doses >550 mg/m2 of doxorubicin, cardiomyopathy. Cyclophosphamide may increase the risk of doxorubicin-induced cardiotoxicity. Dose reduce doxorubicin with liver dysfunction. Hemorrhagic cystitis, dysuria, and urinary frequency. Red-orange discoloration of urine. SIADH. Hyperpigmentation, photosensitivity, nail changes, and radiation recall. Complete alopecia. Hand-foot syndrome can be dose-limiting. Photophobia, increased lacrimation, acute and chronic conjunctivitis, blepharitis, and blurred vision.
- Chair time: 2 hours on day 1. Repeat cycle every 21 days until disease progression.
