Write It Down Documenting mechanical debridement
In your notes, record:
date and time of wound debridement
area debrided
solutions and medications used
wound condition, including signs of infection or skin breakdown
patient's tolerance of and reaction to the procedure
pain assessment
indications for additional therapy.
