Write It Down Documenting catheter irrigation
In your notes, record:
patient education
amount, color, and consistency of return urine flow
patient's tolerance of the procedure
any resistance met during instillation of the solution.
If the return flow volume measures less than the amount of solution instilled, note this on the fluid intake and output balance sheets as well as in your notes.
