Number each fetal monitoring strip in sequence and label each printout sheet with the patient's identification number or birth date, her name, the date and time, and paper speed.

In your notes, record:

• time of vaginal examinations, membrane rupture, drug administration, and maternal or fetal movements

• patient education

• maternal vital signs

• intensity of uterine contractions

• each movement or readjustment of the tocotransducer and ultrasound transducer

• assessments and interventions (in correlation with monitor printout strips).
