Adult Dosing
Major depressive disorder
- Initial doses: 6 mg/24 hr TDP daily
- Titration: Increase 3 mg/24 hr q2 wks if required. Max: 12 mg/24 hr
Notes- Apply to dry intact skin on upper torso/upper thigh/outer surface of upper arm
- Apply patch at approximately the same time each day
- Avoid re-application to the same site on consecutive days
- Do not cut the patch and keep out of the reach of children
Pediatric Dosing
- Safety and effectiveness in pediatric patients have not been established

- Closely monitor patients for clinical worsening, suicidal tendencies and unusual changes in behavior; particularly early in therapy or after increases in dose. Carefully weigh the risks and benefits before starting treatment [US Black Box Warning]
- Therapy is not approved for use in the pediatric patients [US Black Box Warning]
- Apprise families and caregivers of pediatric patients about the emergence of agitation, irritability, unusual changes in behavior, as well as the emergence of suicidality related to drug and to report such symptoms immediately to health care providers
- Monotherapy may lead to precipitation of a mixed/manic episode in patients at risk for bipolar disorder. Adequately screen patients for bipolar disorder prior to initiating treatment
- MAO is important in the catabolism of dietary amines. Hypertensive crisis may occur following the ingestion of tyramine-rich foods
- Therapy can cause postural hypotension, sometimes with orthostatic symptoms
- Use cautiously in patients with disorders or conditions that can produce altered metabolism or hemodynamic responses
