Adult Dosing
Moderately disturbed nonhospitalized patients with schizophrenia
- Initial: 4-8 mg PO tid; reduce as soon as possible to minimum effective dosage
Hospitalized patients with schizophrenia
- 8-16 mg PO bid-qid; Max: 64 mg/day PO
Severe nausea and vomiting
- 8-16 mg/day PO in divided doses; 24 mg occasionally may be necessary; early dosage reduction is desirable
Pediatric Dosing
- Safety and effectiveness in pediatric patients have not been established

- Elderly patients with dementia-related psychosis treated with antipsychotic drugs are at an increased risk of death. Perphenazine is not approved for the treatment of patients with dementia-related psychosis [US Black Box Warning]
- Therapy may cause a potentially fatal symptom complex like Neuroleptic Malignant Syndrome (NMS)
- Adverse events like agitation, hypertonia, hypotonia, tremor, somnolence, respiratory distress and feeding disorder may occur in neonates if administered during the third trimester of pregnancy. Neonates are at risk for extrapyramidal and/or withdrawal symptoms following delivery
- Perphenazine should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus
- Events of leukopenia/neutropenia and agranulocytosis have been reported temporally related to antipsychotic agents

- Obstructive GI disease

- Drug-induced leukopenia or neutropenia history
- Leukopenia

Breastfeeding: Safety Unknown. Perphenazine produced in low levels in milk. No adverse developmental effects have been reported when other phenothiazines are used alone. Monitor the infant for excessive drowsiness during breastfeeding and for developmental milestones, especially if other antipsychotics are used concurrently.
