- Allergic reactions including bronchial asthma have been occurred in susceptible patients
- An increased risk of hyperkalemia has been reported with eplerenone in patients with impaired renal function and diabetics with proteinuria. Appropriate patient selection, monitoring, and avoidance of certain concomitant medications is advised
- Monitor serum potassium at baseline, 1 wk, 1 month, or at dose adjustments and then periodically
- Monitor serum creatinine and CrCl, BP periodically
Cautions: Use cautiously in
- Severe hepatic impairment
- CHF pts post MI if serum creatinine >2 mg/dL (males) or >1.8 mg/dL (females)
- Diabetes mellitus; type 2 microalbuminuria (in CHF pts)
- Diabetic patients with proteinuria (CHF post MI)
Pregnancy Category:B
Breastfeeding: Safety unknown; however eplerenone is excreted in breastmilk of lactating rats. Due to potential for adverse effects on the nursing infant, a decision should be made whether to discontinue nursing or discontinue the drug, (taking into account the importance of the drug to the mother).
