Adult Dosing
Corticosteroid-responsive dermatoses
- Cream (0.025%), ointment (0.025%): Apply to affected areas 2-4 times daily
- Cream (0.1%, 0.5%), lotion (0.1%), ointment (0.1%, 0.5%): Apply to affected areas 2-3 times daily
- Spray: Apply to effected area 3-4 times daily
Note: Occlusive dressings can be used in case of psoriasis or recalcitrant conditions, remove occlusive dressings if infection develops and give appropriate antibiotics

- Systemic absorption can produce reversible hypothalamic-pituitary-adrenal (HPA) axis suppression; potential for glucocorticosteroid insufficiency after withdrawal from treatment. Periodically evaluate for HPA axis suppression if applying to larger surface area and areas under occlusion. Withdraw or substitute with a less potent corticosteroid if HPA axis suppression is noted
- Systemic absorption is increased in inflamed skin, prolonged use, application to large surface area, or use of occlusive dressings
- Discontinue or reduce dose or potency if HPA axis suppression, Cushing's syndrome, hyperglycemia, glucosuria, or irritation occurs
- Pediatric patients are at a greater risk of adrenal insufficiency during or after withdrawal of treatment. Due to a higher ratio of skin surface area to body mass, they are also at a greater risk of HPA axis suppression and Cushing's syndrome than adults
- For topical use only; avoid contact with eyes
- Avoid abrupt cessation in chronic use; may cause infrequent sign and symptoms of steroid withdrawal
- Occlude only if necessary; do not occlude higher potency products
- Perform urinary-free cortisol and ACTH stimulation tests periodically in patients for evidence of HPA axis suppression
- Use lowest effective dose and lower potency formulations
- Institute appropriate antifungal or antibacterial therapy in case of pre-existing skin infection
- Discontinue use if skin irritation develops

- Diabetes mellitus (prolonged use)
- Pre-existing skin atrophy
- Nursing mothers
Supplemental Patient Information
- Avoid inhalation of the spray and protect eyes while applying on face
- Patients should report any signs of local adverse reactions especially under occlusive dressing
- Instruct patient to inform physician if symptoms of underlying disease return or worsen
- Advise parents of pediatric patients not to use this drug on diaper dermatitis or pre-existing skin atrophy
Pregnancy Category:C
Breastfeeding: Safety unknown; no published data; short-term application of topical corticosteroids is not expected to pose a risk to the breastfed infant by passage into breastmilk. Use of the least potent drug on the smallest area of skin of mother is recommended. Ensure that the infant's skin does not come into direct contact with the areas treated. Use of lower potency corticosteroids, water-miscible cream products on the nipple or areola is recommended. Any topical corticosteroid should be wiped off thoroughly prior to nursing if it is applied to the nipple or areola.
