- Tolterodine can cause urinary retention. Use cautiously in patients with clinically significant bladder outflow obstruction
- Antimuscarinic drugs including tolterodine decreases gastrointestinal motility, hence use cautiously in patients with conditions associated with decreased gastrointestinal motility and also in patients with gastrointestinal obstructive disorders because of the risk of gastric retention
- QT prolongation has been reported with tolterodine, exercise caution in patients with a known history of QT prolongation or to patients who are taking Class IA (e.g., quinidine, procainamide) or Class III (e.g., amiodarone, sotalol) antiarrhythmic medications
- Anticholinergic central nervous system (CNS) effects such as dizziness and somnolence may occur with tolterodine. Monitor for signs of anticholinergic CNS effects, especially after starting treatment or increasing the dose. Counsel patients not to drive or operate heavy machinery after initiating tolterodine. Consider dose reduction or discontinuation if a patient experiences anticholinergic CNS effects

Breastfeeding: No information is available on the use of tolterodine during breastfeeding. Long term use might interfere with lactation. Observe the sign of decreased lactation during the long term use. This information is based upon LactMed database (available at http://toxnet.nlm.nih.gov/cgi-bin/sis/htmlgen?LACT last accessed 18 March 2011). Tolterodine is excreted into the milk in mice resulting in the low weight gain in offsprings of mice treated with tolterodine 20 mg/kg/day. It is not known whether excreted in human milk. Manufacturer recommends, decision should be made whether to discontinue nursing or to discontinue drug taking in to account the importance of drug to the mother.
