Adult Dosing
Mild-moderate pain/fever
- 325-650 mg PO q 4 hrs; Max: 4 g/day
Rheumatoid arthritis, Arthritis and Pleurisy of SLE
- Initial dose: 3 g/day PO in divided doses q4-6 hrs
- May increase PRN, with target plasma salicylate level of 150-300 mcg/mL
Juvenile rheumatoid arthritis
- Initial dose: 90-130 mg/kg/day PO in divided doses q4-6 hrs
- May increase PRN, with target plasma salicylate level of 150-300 mcg/mL
Spondyloarthropathies
- Up to 4 g/day PO in divided doses q4-6 hrs
Osteoarthritis
- Up to 3 g/day PO in divided doses q4-6 hrs
Ischemic stroke and TIA
Suspected acute MI
- Initial dose: 160-325 mg PO as soon as MI suspected
- Maintenance dose: 160-325 mg PO qd x 30 days post infarction
Prevention of recurrent MI, Unstable angina pectoris, Chronic stable angina pectoris
CABG
- 325 mg PO 6 hrs post procedure
- Maintenance dose: 325 mg PO qd
PTCA
- Initial dose: 325 mg PO 2 hrs pre-surgery
- Maintenance dose: 160-325 mg PO qd
Carotid Endarterectomy
- 80 mg qd-650 mg bid PO, start pre-surgery

- Each dose should be administered with glass full of water, unless patient is fluid restricted
Acute Coronary Syndromes [Non-FDA Approved]
- Loading dose 300325 mg PO, then 75-100 mg daily
Kawasaki Diseases [Non-FDA Approved]
- 80-100 mg/kg/day PO divided qid, given either until day 14 of illness or until afebrile for 48-72 hours
- Continued at 35 mg/kg PO qd for 68 wks. May be discontinued if there is no evidence of coronary abnormalities

Pediatric Dosing

- Aspirin can cause Reye's syndrome when used in children and teenagers who have, or are recovering from chicken pox or flu-like symptoms, if changes in behavior with nausea and vomiting occur, discontinue the drug and provide appropriate therapy as this can be the early sign of Reyes syndrome
- Severe allergic reactions are associated with use of aspirin. Blisters, facial swelling, hives, rash, shock and skin reddening are the associated symptoms. Discontinue the treatment at the first appearance of skin rash or any other sign of hypersensitivity like anaphylactoid reactions
- Aspirin inhibits platelet function leading to increase in bleeding time. Hence use cautiously in patients with bleeding disorders
- Aspirin causes inflammation, bleeding, ulceration, and perforation of the stomach. Restrict use in patients with a prior history of peptic ulcer disease or gastrointestinal bleeding, taking blood thinning (anticoagulant) or steroid drug. If a serious GI adverse event is suspected, discontinue the therapy and promptly institute an alternative therapy
- Avoid long term therapy with the drug, concomitant use of oral corticosteroids or anticoagulants, smoking, use of alcohol
- Do not administer in pregnant female during third trimester as it causes premature closure of the ductus arteriosus in the fetus
- Advise patients to stop the use of drug and inform their clinician if they experience bloody vomit, bloody or black stools, stomach pain that does not get better, redness or swelling, ringing in the ears, or loss of hearing

Breastfeeding: It is best to avoid aspirin during breastfeeding, especially with very young infants. As per some expert opinion, low dose 75-162 mg of aspirin can be used as an antiplatelet drug in nursing mothers, avoid chronic high dose. Avoid breastfeeding for 1 to 2 hours after a dose, if aspirin is to be taken, to minimize antiplatelet effects in the infant. The risk of Reye's syndrome caused by salicylate in breastmilk is unknown. Manufacturer advises avoiding aspirin use in nursing mothers as salicylate is excreted in breast milk.
