- Never administer in alcohol intoxication or without patient's full knowledge; the physician should instruct relatives accordingly [US Black Box Warning]
- Used as an alcohol deterrent to aid in the management of alcohol dependence; the drug is not a cure for alcohol dependence, and disulfiram therapy should be confined to selected, highly motivated patients in conjunction with supportive psychotherapy
- Patient should be fully informed of the disulfiram alcohol reaction
- Ingestion of alcohol during treatment will produce symptoms like flushing, throbbing in head and neck, respiratory difficulty, nausea, vomiting, sweating, thirst, chest pain, palpitation, dyspnea, hyperventilation, tachycardia, hypotension, syncope, marked uneasiness, weakness, vertigo, blurred vision, confusion and also severe reactions like respiratory depression, cardiovascular collapse, arrhythmias, myocardial infarction, acute congestive heart failure, unconsciousness, convulsions and death depending upon the quantity of alcohol in the body
- Before starting therapy, the patients with history of rubber contact dermatitis should be evaluated for hypersensitivity to thiuram derivatives
- Avoid exposure to ethylene dibromide or its vapors
- Perform LFT's at baseline and after 10-14 days and then periodically during treatment
- Monitor CBC, serum chemistries

- Renal impairment
- Hepatic impairment
- Diabetes mellitus
- Hypothyroidism
- History of seizure disorder
- Cerebral damage
Supplemental Patient Information
- Full education of the consequences of ingestion of even small amounts of alcohol should be fully understood
- Effects may occur up to 2 weeks after discontinuation
- Notify the physician immediately if any symptoms of hepatitis, such as fatigue, weakness, malaise, anorexia, nausea, vomiting, jaundice, or dark urine
- Carry identification card stating patient is on disulfiram treatment and describing the symptoms most likely to occur as a result of the disulfiram-alcohol reaction and the contact number of the physician and institution to be contacted in an emergency
