Before any Pt can be transferred to OR:

• Anesthesia must conduct an in-person interview and assessment, which includes written pre-op anesthesia orders—orders must be completed prior to going to OR.
• Surgeon must conduct an in-person conversation with Pt; any final questions must be addressed; surgeon verifies with Pt and marks correct surgical site with their initials using permanent marker (some facilities may require Pt to also mark surgical site); surgeon verifies there have been no changes since original pre-op consultation and H&P were completed—if >30 days, a new H&P must be done.
• Pre-op nurse and OR staff (circulator and/or anesthesia) must simultaneously be present for a final safety check and time-out (see Time-Out/Pause for Cause).
