Adult Dosing
Reduce risk of atherosclerotic events (MI, CVA, PVD)
- 75 mg PO qd, with or without food
Acute coronary syndrome (unstable angina/Non-Q-wave MI)
- 300 mg oral loading dose, then 75 mg PO qd; give along with aspirin 75-325 mg PO qd
Acute coronary syndrome (STEMI)
- 75 mg PO qd in combination with aspirin 75-325 mg PO qd, with or without thrombolytics
Acute Coronary Syndromes [Non-FDA Approved]
- Loading dose: 300 600 mg PO
- 75 mg PO qd for upto 12 months following treatment of UA/NSTEMI, (also advised following insertion of most drug eluting stents)
Pediatric Dosing
- Safety and effectiveness in pediatric population have not been established

- Concurrent use with drugs known to inhibit CYP2C19 may reduce levels of active metabolite and subsequently reduce clinical efficacy; avoid concurrent use of strong or moderate CYP2C19 inhibitors [US Black Box Warning]
- Consider testing for CYP2C19 genotype before starting therapy; consider alternative treatment if identified as poor metabolizers [US Black Box Warning]
- Combination of aspirin and clopidogrel may increase the risk of major bleeding especially in patients with recent TIA or stroke
- Thrombotic thrombocytopenic purpura (TTP) has been reported rarely following use of clopidogrel; may occur sometimes after short-term exposure (less than 2 wks)
- Monitor patient for bleeding or unusual bruising
- Avoid lapses in therapy; if temporarily discontinued, restart as soon as possible. Increased risk of cardiovascular events if discontinued prematurely
- Clopidogrel prolongs the bleeding time and therefore should be used with caution in patients who may be at risk of increased bleeding (trauma, surgery, or other pathological conditions). If undergoing surgery, discontinue clopidogrel 5 days prior to surgery
- Patients should be assessed for history of hypersensitivity to another thienopyridine since allergic cross-reactivity among thienopyridines has been reported
Cautions: Use cautiously in
- Hepatic impairment
- Elective surgery within 5 days
- Ocular disease
- Intermediate CYP2C19 metabolizer
Pregnancy Category:B
Breastfeeding: Safety unknown; excreted in the milk of rats; Because of the potential for possible serious adverse reactions in nursing infants, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
