Adult Dosing
Treatment of patients with HIV infection
- 800 mg PO q8 hrs with other antiretroviral agents
- Note: For adequate hydration, drink at least 1.5 liters (approximately 48 ounces) of liquids during the course of 24 hours
Pediatric Dosing
Treatment of patients with HIV infection
- The optimal dosing for children has not been established and is inadequate; however, some prescribers use dosing of:
- Child 3 month-3 years: 250-500 mg/m2/dose PO q8 hrs
- Child 3-18 years: 500-600 mg/m2/dose PO q8 hrs

Renal Dose Adjustment
- Renal impairment: Dose adjustments not defined
Hepatic Dose Adjustment (Adult)
- Mild-to-moderate hepatic insufficiency resulting from cirrhosis: 600 mg PO q8 hrs.
- Severe impairment: Dose adjustments not defined

- Administer on an empty stomach, 1 hrs before or 2 hrs after a meal
- Maintain adequate hydration to avoid risk of nephrolithiasis
- Risk is substantially higher in pediatric patients versus adults for nephrolithiasis/urolithiasis; discontinue or suspend therapy during acute Nephrolithiasis/Urolithiasis
- Monitoring is required in case of renal impairment
- Combination antiretroviral therapy has been associated with the redistribution of body fat (lipodystrophy) in HIV patients
- Acute hemolytic anemia, including cases resulting in death, has been reported in patients treated with indinavir; discontinue if diagnosed
- Patients may develop immune reconstitution syndrome resulting in the occurrence of an inflammatory response to an indolent or residual opportunistic infection; further evaluation and treatment may be required
- Tubulointerstitial nephritis in patients with severe leukocyturia has been reported and monitoring is recommended
- Use with atazanavir, lovastatin, rifampin, rosuvastatin, salmeterol, simvastatin, St John's Wort is not recommended
- Concomitant use with a potent CYP3A4 inhibitor is not recommended

Supplemental Patient Information
- Patients should be advised to report use of any other medication or herbal products, particularly St. Johns wort to their doctor as indinavir may interact with some drugs
- Instruct patient to report severe nausea or diarrhea, fever, chills, flank pain, urine or stool color changes, yellowing of skin or eyes
- If GI upset occurs, advise patient to take with a light meal
Pregnancy Category:C
Breastfeeding: Unsafe; women with HIV should not breastfeed their infants to avoid risking postnatal transmission of HIV.
