Adult Dosing
Management of chronic stable angina
Immediate-release form
- 30-90 mg PO qid; Start: 30 mg PO qid, incr. 30 mg/dose q1-2 days
- Max: 360 mg/day
24h extended-release form
- 120-480 mg ER PO daily; Start: 120-240 mg ER PO daily, titrate over 7-14 days
- Max: 540 mg/day ER

Hypertension
12h extended-release form
- 120-180 mg ER PO bid, titrate over 7-14 days; Start: 60-120 mg ER PO bid
- Max: 360 mg/day ER

Pediatric Dosing
- Safety and effectiveness in pediatric patients has not been established

- Prolongation of AV node refractory periods without significantly affecting sinus node recovery time is associated with this drug. Abnormally slower heart rates (particularly in patients with sick sinus syndrome) or second or third-degree AV block may occur in patients. Additive effects on cardiac conduction may occur on concurrent use of diltiazem with beta-blockers or digitalis
- Patients with preexisting impairment of ventricular function are prone to worsening of CHF. Exercise caution while using this drug either alone or in combination with beta-blockers in patients with impaired ventricular function
- Decrease in blood pressure occurred in association with use of this therapy may occasionally result in symptomatic hypotension
- Rare occasions of significant elevations in enzymes such as alkaline phosphatase, LDH, SGOT, SGPT, consistent with acute hepatic injury have occurred
- Monitor renal and hepatic function at regular intervals
- Transient dermatological events have occurred which may disappear despite continuous use of this drug
- Occasions of skin eruptions progressing to erythema multiforme and/or exfoliative dermatitis have occurred. Discontinue therapy on persistence of dermatologic reactions
Caution: Use cautiously in
- Renal impairment
- Hepatic impairment
- Cardiac conduction defects
- Left ventricular dysfunction
- Patients with preexisting severe GI narrowing (pathologic or iatrogenic)
Pregnancy Category:C
Breastfeeding: Based on literatures available amounts of diltiazem ingested by the infant are low and are unexpected to cause any adverse reactions in breastfed infants. According to manufacturer's data if use of this drug is deemed essential institute an alternative method of infant feeding.
